Johnny Frank was born October 14, 1945, to James Junior Eaves and Hilda Hall Eaves in Louisville. He is survived by three brothers: Ronald Eaves and wife Pat of Terry, Mississippi; Don Eaves and wife Gail of Cleveland, Georgia; and Vance Eaves and wife Jenny of Louisville. Johnny Frank is preceded in death by his parents.
